Members of KDN's Health Plan Partnership workgroup have compiled Type 2 Diabetes Algorithim tools from national resources to assist clinicians in applying diabetes treatments. Member partners of the Kentucky Diabetes Network, Inc. have developed several tools to assist health care professionals screen, track or manage their patients with or at risk for diabetes. Another useful tool, developed by our partners is the My Personal Diabetes Health Card. This wallet card is to be used by individuals with diabetes.
The Pre-Diabetes Risk Screening and Tracking Tool, the Diabetes Care Tool and the Personal Diabetes Health Card indicate minimum services to be provided in the continuing care of adults with diabetes. Members of KDN's Patient Education Workgroup adopted an 11" x 17" poster from the National Diabetes Education Program that serves as a colorful reminder of what you and your doctor can do to help you control your diabetes. In addition to these practical tools, KDN's Patient Education Work Group compiled a resource tool for use with your diabetes population. The work group requested lists of free or low-cost materials developed by a variety of companies and put the list together into a "Guide to Diabetes Educational Materials". Another addition to our Diabetes Care Tools resource inventory is our eye brochure developed by KDN's Health Plan Partnership. This informative brochure discusses the eye problems that people with diabetes are at risk for developing. Diabetes Care Tools
These tools, as well as, educational brochures, standing orders and other resource materials that are developed by KDN; are updated annually to follow the American Diabetes Association's current "Standards of Care for Patients with Diabetes Mellitus".
