Health Plan Partners:

The Health Plan Partnership (HPP) committee is made up of representatives from the major healthcare entities and payers in Kentucky and Southern Indiana, including but not limited to The Department for Medicaid Services, Health Care Excel, Anthem Blue Cross Blue Shield, Bluegrass Family Health, UnitedHealthcare of Kentucky, and Humana Inc.

The Committees goal is to adopt and deliver the same key message to its overlapping network of providers and members by promoting the use of the ADA Guidelines.

Projects include

1. An eye care campaign distributing 300,000 eye care stickers to pharmacy clients with diabetes
   
   

2. Developed a Diabetes Care Tool (flow sheet) intended for use by healthcare providers to document the minimum services as recommended by the American Diabetes Association for adults with diabetes
   
   

3. This committee organized and distributed 2,000 Provider Packets to physician offices. The packets contained information about The KDN, helpful tools such as The Diabetes Care Tool, patient wallet card, a 4-page resource guide of free diabetes materials; “high-risk feet” chart labels and foot exam form, and a reference sheet of billing codes commonly used for diabetes.
