Sharing LIGHT and HOPE for People with or at Risk for Diabetes
Patient Education:
Priority purpose is to
increase the number of individuals diagnosed with diabetes to have at least one and preferably more A1c tests annually
to increase wellness care for individuals with diabetes
To increase the number of foot, eye & kidney exams obtained by people with diabetes.
Target audience: individuals with diabetes.
Activities include:
Annual diabetes expo, the annual diabetes expo is conducted in Louisville, KY targeting the high-risk population of African Americans. Multiple educational opportunities as well as 20 diabetes organization booths are available at the expo.
Creation /distribution of Diabetes Wallet Cards, the Diabetes Wallet Cards include valuable information that the persons with diabetes can use to track their care in one convenient location.
The Guide to Diabetes Educational Materials was created to serve as a quick reference to free or low-cost diabetes education materials.
16,000 posters with the message “If you have diabetes you can be in control” were distributed to pharmacies, physician offices, churches, grocery stores and multiple other sites across the state.
