Eye Brochure Developed

KDN's Health Plan Partners developed an eye brochure that includes a form for obtaining the test results from eye care providers. This informative brochure, discusses eye problems that people with diabetes are at risk for developing. The form, when completed by the eye care provider can be returned to the primary medical provider and/or diabetes educator.

Numerous Health Care Plans across Kentucky plan to utilize this brochure with distribution to their respective diabetes populations.
