Mission Statement 

The Kentucky Diabetes Network, Inc. (KDN), a network of public and private partners, is dedicated to improving the treatment and outcomes for Kentuckians with diabetes, to promote early diagnosis of diabetes, and ultimately to prevent the onset of diabetes.

Vision

The Kentucky Diabetes Network is the collaborative driving force behind the prevention, control, and eradication of diabetes in Kentucky.

Strategic Goals Summary

The Plan Goals are to:

  • Increase the percentage of Kentuckians recognizing diabetes as a serious disease.

  • Increase wellness care for individuals with and at risk for diabetes.

  • Increase practitioners’ diabetes knowledge and awareness.

  • Increase the number of foot, eye, and kidney exams obtained by people with diabetes.

  • Ensure that all persons with newly diagnosed diabetes have access to quality self-management programs.

  • Promote cultural competence and diversity among KDN members and Kentuckians.

  • Promote funding and research for the control, prevention, and care of diabetes.

  • Foster cooperative efforts to increase diabetes screening, especially for at-risk populations.

  • Increase the number of individuals with diabetes who have hemoglobin A1C tests annually.

  • Advocate and foster changes in policies and plans which ensure non-discrimination for people with diabetes.

  • Serve as a catalyst for coordinated statewide implementation of the Strategic Plan.
